Requirement:
We provide services to patients admitted in the hospital or those referred from outside the hospital at the WCH Radiology department. These patients can be state or private patients. Every patient that has need for our services SHOULD be referred to us by a registered Medical Doctor or specialist using relevant request forms.
There are yellow x-ray request forms or white whenever photocopied. Most of the x-ray procedures are requested on this form besides request for ultrasounds, computed Tomography and Catheterization procedures. Occasionally the relevant forms may not be available, but we strongly discourage for patients to be referred using any other form. The patient needs the request form as their health passport for every visit to the radiology department.
It is essential to remain motionless during the x-ray, since movement causes the resulting picture to be blurry. Sometimes patients are ask to hold their breath briefly during the procedure.
Process:
Patients need to consult a general practitioner or a specialist in order to be referred to for any X-ray procedure as the Dr. See fix. X-ray cannot be taken without a valid x-ray, CT scan or ultrasound request form from the doctor/specialist.
General x-ray, ward radiography and call outs (afterhours) are done on the same day when pit is requested and only the request form is needed. But for CT Scans, Mammography IVP, Catheterization, Fluoroscopy and Ultrasound procedures patients will be booked and given a date on which the examination will be done.
Patients might need to bring blood or any other results when they come for the scheduled radiology procedures (examination) such as for:
CT Scan: patients will be need to produce the request form , urea and Electrolyte ( U&E) blood results and an informed consent form signed by the patient, referring physician and a witness.
Fluoroscopy: Patients will need a request form and consent form.
Mammography: patients will need only request form, but for special mammography procedures a consent is also needed.
IVP: Patients will need a request form, signed informed consent form U&E blood results and a running intravenous line (IV).
Catheterization: Doctors write in the patients’ health passport and referee them to the cardiologist.
Fluoroscopy: The patient will need a request form, ultrasound results and or general x-rays for some procedures.
Ultrasounds: Previous examinations, x-ray films, CT scan images, blood results and a signed consent form for some investigations should be accompany the patient.
